Your privacy is important and, with few exceptions (see client/counselor agreement), I will not contact anyone without your permission. However, it is often helpful for me to communicate with doctors, family members, or other important people in your life. We will use this form if you and I decide it would be helpful for me to talk to someone about your therapy.
